While awareness of blood glucose control, nutrition, and lifestyle is improving in some regions, diabetes prevalence continues to rise everywhere. Over the next two decades, cases in low- and middle-income countries are projected to increase by 45-145% (<a href=\"https:\/\/diabetesatlas.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">The Diabetes Atlas<\/a>).<\/p>\n\n\n\n<p>Beneath the surface lies another side of the disease, one that\u2019s hidden, invisible to the naked eye, but crucial to understanding diabetes and its complications. The world of <strong>microcirculation<\/strong>: the vast network of tiny blood vessels that provide healing oxygen and nutrients to every cell and tissue, and just about every organ of the body.<\/p>\n\n\n\n<p>In this post, I discuss why microcirculation matters in the context of diabetes, how early changes can serve as warning signs, and share the story of one woman whose microcirculation measurements changed the fate of her foot.<a id=\"_msocom_1\"><\/a><\/p>\n\n\n\n<p><strong><a href=\"#lx-bio\">\/\/Liuting Xu<\/a><\/strong><\/p>\n<\/div>\n<\/div>\n\n\n<div class=\"gn-spacer-block pb-12\">\n    <\/div>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\"><div class=\"block-img relative  pt-2 md:pt-16 \" >\n                                        <img decoding=\"async\" loading=\"lazy\" src=\"https:\/\/www.perimed-instruments.com\/us\/wp-content\/uploads\/sites\/3\/periflux-6000-epos-how-it-works-skin.webp\" alt=\"\"\n                    width=\"300\" height=\"1000\" class=\" has-round-corners\">\n                                <\/div>\n            <\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:66.66%\">\n<h5 class=\"wp-block-heading\" id=\"h-why-microcirculation-matters\">Why microcirculation matters<\/h5>\n\n\n\n<p>In a healthy person, the microcirculation continuously adapts to local demands, providing oxygen and nutrients as needed. Diabetes gradually disrupts this finely tuned system, causing endothelial dysfunction, reduced perfusion, slower wound healing, and greater susceptibility to infection.<\/p>\n\n\n\n<p><strong>Microvascular dysfunction<\/strong> underlies many of the most serious complications of diabetes \u2014 foot ulcers, retinopathy, and neuropathy \u2014 yet it often remains undetected until significant damage has occurred.<\/p>\n\n\n\n<p>By the time more familiar signs of diabetes appear, such as blurry vision, extreme fatigue, or changes in skin color, microvascular damage may already be advanced. Without the right diagnostic tools, these early changes remain invisible.<\/p>\n\n\n<div class=\"gn-spacer-block pb-4\">\n    <\/div>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-why-microcirculation-isn-t-enough\">Why microcirculation isn\u2019t enough<\/h5>\n\n\n\n<p>Standard diagnostic tools focus on macrocirculation and include ultrasound, ankle-brachial index (ABI), and\/or toe-brachial index (TBI). While valuable, these measurements don\u2019t detect small-vessel pathology that may be present \u2014 even before macrovascular issues arise.<\/p>\n\n\n\n<p>A patient with diabetes may have a normal ABI yet impaired microvascular perfusion in the foot \u2014 a mismatch that may not become apparent until an ulcer develops.<\/p>\n\n\n\n<p>People with diabetes often have incompressible arteries due to medial arterial calcification \u2014 calcium buildup in the middle layer of arterial walls \u2014 which stiffens the arteries and produces falsely elevated ABI values.<\/p>\n\n\n\n<p><a id=\"_msocom_1\"><\/a><\/p>\n<\/div>\n<\/div>\n\n\n<div class=\"gn-spacer-block pb-12\">\n    <\/div>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:66.66%\">\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<figure class=\"wp-block-pullquote has-base-font-size\"><blockquote><p><\/p><cite>All patient data presented here is anonymized and used in compliance with applicable privacy regulations. No information that can identify the patient or clinical site has been disclosed.<\/cite><\/blockquote><\/figure>\n<\/blockquote>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-a-limb-preserved\">A limb preserved<\/h5>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><\/p>\n<\/blockquote>\n\n\n\n<p>Behind every data point is a person \u2014 someone whose quality of life can take a dramatic turn with the proper assessment and treatment.<\/p>\n\n\n\n<p>One such case, observed during a clinical demonstration of PeriFlux 6000, involved a middle-aged woman with diabetes and comorbid conditions, including a right foot ulcer.<\/p>\n\n\n\n<p><strong>Step 1 \u2014 macrovascular assessment<\/strong><br>Her ABI and TBI values were within reference ranges.&nbsp;<\/p>\n\n\n<div class=\"gn-spacer-block pb-4\">\n    <\/div>\n\n\n<div class=\"block-img relative max-w-full  \" >\n                                        <img decoding=\"async\" loading=\"lazy\" src=\"https:\/\/www.perimed-instruments.com\/us\/wp-content\/uploads\/sites\/3\/pvr.webp\" alt=\"\"\n                    width=\"2098\" height=\"637\" class=\"w-full has-round-corners\">\n                                <\/div>\n            \n\n<div class=\"gn-spacer-block pb-4\">\n    <\/div>\n\n\n\n<p><strong>Step 2 \u2014 microvascular evaluation with TcpO2<\/strong><br>Given the location of the foot ulcer and marked skin discoloration, TcpO2 was measured at two sites on the dorsal surface of her right foot.<\/p>\n\n\n<div class=\"block-img relative max-w-full  \" >\n                                        <img decoding=\"async\" loading=\"lazy\" src=\"https:\/\/www.perimed-instruments.com\/us\/wp-content\/uploads\/sites\/3\/TcpO2.webp\" alt=\"\"\n                    width=\"1500\" height=\"500\" class=\"w-full has-round-corners\">\n                                <\/div>\n            \n\n<div class=\"gn-spacer-block pb-4\">\n    <\/div>\n\n\n\n<p>The baseline for site #1 is just below the impaired-healing threshold of 40 mmHg. On oxygen challenge, her levels improved by 10mmHg and 15 mmHg. The hospital\u2019s guidelines, based on Dr. Caroline Fife\u2019s findings <a href=\"#h-references\"><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-btn-color\">[1]<\/mark><\/a>, state that if the increase during an oxygen challenge is below 10 mmHg, amputation healing failure is likely without an intervention.<\/p>\n\n\n\n<p><strong>Step 3 \u2014 additional microcirculation insight with SPP<\/strong><br>Skin perfusion pressure (SPP) was then measured at three levels on each foot \u2014 lateral, medial, and toe.<\/p>\n\n\n<div class=\"gn-spacer-block pb-4\">\n    <\/div>\n\n\n<div class=\"block-img relative max-w-full  \" >\n                                        <img decoding=\"async\" loading=\"lazy\" src=\"https:\/\/www.perimed-instruments.com\/us\/wp-content\/uploads\/sites\/3\/ankle-and-toe-pressure.webp\" alt=\"\"\n                    width=\"1700\" height=\"900\" class=\"w-full has-round-corners\">\n                                <\/div>\n            \n\n<div class=\"gn-spacer-block pb-4\">\n    <\/div>\n\n\n\n<p>With all SPP values above 40 mmHg, in combination with her TcpO2, ABI, and TBI results, the clinician decided not to proceed with amputation.<\/p>\n\n\n<div class=\"gn-spacer-block pb-12\">\n    <\/div>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"h-seeing-what-s-hidden\"><strong>Seeing what\u2019s<\/strong> <strong>hidden<\/strong><\/h3>\n\n\n\n<p>This case highlights how microcirculation assessment can inform clinical decisions and change patient outcomes.<\/p>\n\n\n\n<p>On World Diabetes Day, we are reminded that diabetes is more than a metabolic disease characterized by chronic hyperglycemia.
